Community
Re-Entry

your own home. Many people in nursing homes are able and
want to live in the community.
Until now, the
largest barriers to moving out of a nursing facility were
money and support. RAMP will help qualified individuals, under
60 years of age, to transition successfully into an
independent living situation by providing support for housing,
equipment, remodeling, initial utility and rent down payments.

Information
and Referral
RAMP is the local point of
contact for all people with disabilities, their families and
the community to find appropriate resources and information on
disability issues.
RAMP will provide you
with specific information on accessibility requirements
and can assist builders and remodelers to make commercial and
residential buildings accessible.
Peer
Support
Working with others who have "been there - done
that" is invaluable. RAMP can help you find a one-on-one
or group support situation with people with similar
disabilities.
Many people with
disabilities are overwhelmed trying to find out information
about their disability and how to not only cope with their
situation but thrive and excel.
Personal
Assistant Services
Personal Assistants (PAs) are
essential for many people to live independent lives. PAs
assist with
essential tasks of daily living that a person with a
disability cannot
do themselves. Frequently
asked
questions.
RAMP can help you
find and manage a PA. We maintain a list of people interested
in working as PAs. Contact RAMP's Personal Assistant
Coordinator for information on obtaining a PA, becoming a PA,
or for PA training schedules.
Youth
Education Advocacy
RAMP provides support for students and their parents working
through disability related education issues. We also provide
planning for the transition from school to life after
school.
RAMP will help you

with a disability and how to enforce every student's
right to an appropriate education.
